(1) 24-hour mobile crisis teams, (2) emergency crisis intervention services, (3) crisis stabilization services, (4) suicide crisis response; and (5) services for substance abuse crisis and intoxication, including ambulatory and medical withdrawal management. "Crisis services are provided by C-Trilogy directly and, when needed, by an existing state-sanctioned, certified, or licensed system or network.
Crisis Walk-in Services
Walk-in crisis services are office-based and triaged by a QMHP providing immediate screening and assessment and brief, intensive interventions focused on resolving a crisis and preventing admission to a more intensive level of care. Walk-in crisis services are immediately accessible and serve two purposes:
• Ready access to psychiatric assessment and treatment for new individuals with urgent needs, and
• access to same-day psychiatric assessment and treatment for existing individuals within the system with urgent needs

Outpatient Behavioral Health services are provided by C-TRILOGY and designed to treat a consumer’s mental health and/or substance use disorder in a manner consistent with the consumer’s phase of life and development; specifically, considering children, adolescents, transition-age youth, and older adults as distinct groups for whom life stage and functioning may affect treatment. C-TRILOGY’s services prioritize and provide care to those living with serious psychotic disorders, severe mental illnesses that result in danger to self/others, and/or grave disability. Services include emergency assessment and treatment including use of appropriate psychotropic medications and psychotherapeutic interventions, ACT and, if so ordered, AOT services.

C-Trilogy directly provides person-centered and family-centered treatment planning. Person and family-centered treatment planning is a collaborative process-directed between C-TRILOGY and the consumer receiving care that results in the development of treatment goals and provision of services. Treatment plans are strength-based and identify individual needs, goals, preferences, capacities, and desired outcomes.

C-Trilogy is partnering with the local VA Clinic to provide intensive, community-based behavioral health care for qualifying members of the U.S. Armed Forces and veterans, particularly those Armed Forces members located 50 miles or more (or one hour's drive time) from a Military Treatment Facility (MTF) and veterans living 40 miles or more (driving distance) from a VA medical facility. These services are provided onsite at C-Trilogy’s offices.
Services provided to veterans is consistent with minimum clinical mental health guidelines promulgated by the Veterans Health Administration (VHA), including clinical guidelines contained in the Uniform Mental Health Services Handbook of such Administration.

Peer/Family/Caregiver Support services are peer-delivered services with a rehabilitation and recovery focus. Peer/Family/Caregiver Support staff have lived experience and are a support to persons receiving services. Peer/Family/Caregiver Support services are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles (e.g., hope and self-efficacy, and community living skills). Peer/Family/Caregiver Support uses trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder.

C-Trilogy directly targets individuals with serious mental illness (SMI) or substance use disorders (SUD); children and adolescents with serious emotional disturbance (SED); and individuals with co-occurring mental and substance disorders (COD).
C-Trilogy’s Targeted Case Management is responsible for high quality services that will assist individuals in sustaining recovery, and gaining access to needed medical, social, legal, educational, and other services and supports. The services are intended to support the wellness and recovery goals of person receiving services with complex and/or chronic behavioral health issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care.

C-TRILOGY is responsible for outpatient clinic primary care screening and monitoring of key health indicators and health risks and that care is coordinated. Primary care screening is a basic physical assessment that measures specific health indicators and needs for a physical exam or further evaluation by appropriate health care professionals. Health monitoring is the continued measuring of specific health indicators associated with increased risk of medical illness and early death. Prevention is a key component of the outpatient clinic primary care screening and monitoring of key health indicators and health risk services. C-Trilogy has a standing clinic offering health care services to persons uninsured and underinsured.

C-Trilogy provides Psychiatric Rehabilitation Services (PRS) designed to provide rehabilitation support, recovery-oriented activities, interventions, and skill development necessary for the individual to improve self-management of and reduce relapse to substance use, the negative effects of psychiatric, or emotional symptoms, that interfere with an individual’s daily living skills that are critical to remaining in home, school, work, and community.

To ensure that C-TRILOGY’s licensed professionals appropriately screen, assess and diagnose consumers with behavioral health disorders for their optimal success and to provide the foundation for treatment and services while utilizing standardized, validated evidence-based screening and assessment tools with developmentally, culturally, and linguistically appropriate measures, and where appropriate, motivational interviewing (MI) techniques.

Centauri Rx is our partnered on-site pharmacy that is conveniently located right inside our office. This allows our patients to truly get a start to finish holistic care experience all in one place.

Centauri Rx accepts all major government and private insurances just like us.

Centauri Rx is a fully functioning independent pharmacy. They are able to fill your prescriptions from our providers as well as other providers you may have prescribing for you. Meaning you can make Centauri your home for all you prescription needs.

Centauri Rx has multiple ways to make sure you have timely access to your medications. They are equally as dedicated as we are to breaking down barriers for access to care. At Centauri you are able to pick up your medications in person at the physical location, or request your medications be delivered to your home to save a trip.

The pharmacist does in person and phone consultations. If you ever have concerns about your medications or have questions about interactions, side effects, or the best way to take your new medications Dr. Wu is always happy to educate and inform his patients.

Even if you are not a patient of ours at C-Trilogy you can still use Centauri Rx as your pharmacy. Make it easy on you and your family and be able to pick up all you medications for the whole family in one stop. No more going to multiple pharmacies for different insurances and medication availability.

Certain insurances require prior authorization for certain medications. Centauri Rx goes above and beyond the average pharmacy and works with our clinical staff to help you get coverage for your important medications.

Certain manufacturers for your medications have co-pay assistance programs. The pharmacist is able to find available programs and help you apply. Centauri Rx staff is always looking for ways to make sure that affordability isn’t ever the reason you aren’t able to access your prescriptions.
